BADGE REQUEST FORM
This is where you will submit your final roster. Before filling this out, you will need to know the first and last name of each Delegate, Associate Delegate, and Advisor. Additionally, you will need to indicate which Delegates are Bill Sponsors.
Shortly after completion, you will receive a copy of your submission to your email address. Please carefully review and inform us of any spelling mistakes or other errors as name badges will be printed from this list.
